Hot Topic: Topika – Finalisierung der "Guideline on quality and equivalence of locally applied, locally acting cutaneous products“ und Auswirkungen auf Produktentwicklung und Lifecycle-Management
Im Oktober 2024 wurde die finale Fassung der “Guideline on quality and equivalence of locally applied, locally acting cutaneous products“ (EMA/CHMP/QWP/708282/2018) veröffentlicht. Damit herrscht sowohl für pharmazeutische Unternehmen als auch für Zulassungsbehörden mehr Klarheit, nachdem die Guideline zuvor über mehrere Jahre nur als Entwurf vorlag.
Neben der Tatsache, dass der Titel der Guideline angepasst wurde und auf lokal angewendete und lokal wirkende Produkte beschränkt wurde (als Abgrenzung zu systemisch wirkenden Produkten), fällt insbesondere auf, dass das Kapitel 5 – das Thema therapeutische Äquivalenz betreffend – überarbeitet wurde. Hier wurden z.B. mehrere Entscheidungsbäume ergänzt, die ein systematisches und schrittweises Vorgehen ermöglichen sollen, sowie weitere Inhalte präzisiert.
Allerdings haben zahlreiche fachliche Kommentare keine ausreichende Berücksichtigung gefunden. So sind weiterhin relativ strenge Akzeptanzkriterien zum Nachweis der Äquivalenz vorgesehen und es wurde auch keine Harmonisierung, z.B. mit FDA-Vorgaben oder Methoden, erreicht. Für die betreffenden Darreichungsformen, die häufig als speziell und komplex angesehen werden, werden sich erhebliche Auswirkungen auf die praktische Handhabung ergeben. Es ist auch zu befürchten, dass das eigentliche Ziel der Guideline, Anreize für den Verzicht auf Durchführung klinischer Endpunktstudien („Therapiestudien“) zu setzen und therapeutische Äquivalenz beispielsweise auch auf Basis von in-vitro oder ex-vivo-Untersuchungen abzuleiten, möglicherweise nicht erreicht werden kann.
Insgesamt werden pharmazeutische Unternehmen sowohl im Rahmen der Produktentwicklung als auch im Hinblick auf das spätere Lifecycle-Management mit zahlreichen Herausforderungen konfrontiert werden. Dabei ist zu erwarten, dass der Zeit- und Kostenaufwand für die einzelnen Prozesse weiter steigen wird. Dies soll im Webinar näher dargelegt werden.
